Community Memorial Hospital is the local health system of choice for quality health care, ensuring local access in collaboration with our local and regional partners.

History

In July 1958, CMH opened its doors, bringing to life a dream shared by neighbors, families, and local businesses. Together, they raised funds to build a hospital, guided by the belief that local healthcare is as essential as fire and police protection.

More than six decades later, a greatly expanded CMH still stands proudly as your independent community hospital. Recognized as a Critical Access Hospital (CAH) by the Centers for Medicare & Medicaid Services, CMH continues to provide the essential care that rural Carlton County depends on.

Community Health Needs Assessment

To fulfill our mission and vision as a nonprofit healthcare facility and meet the requirements enacted by the 2010 Patient Protection and Affordable Care Act, CMH conducts a Community Health Needs Assessment (CHNA) once every three years.

A CHNA is a review of current data, resources, initiatives, gaps, and limitations to identify areas of improvement among our services and partnerships. To view our past and current CHNA, please click below.

MN Rural Obstetrics Simulation & Education Program

In partnership with the Minnesota Department of Health (MDH), Community Memorial Hospital has established the Minnesota Rural Obstetrics Simulation & Education Program (MN ROSE) to improve outcomes for moms and babies across the state of Minnesota. Utilizing some of the best technology and tools available on the market, and subsidized through the generosity of federal, state, and private grants, we can offer valuable training at an affordable cost.
