In alignment with our mission of caring for our neighbors, CMH is committed to providing financial assistance to patients through our Community Care Program.
Our Community Care Program is available for emergency and other medically necessary care provided by CMH to uninsured patients who meet income and asset limit requirements. All patients who believe they may qualify for financial assistance are encouraged to submit a Community Care Program application.
Some providers and services may not be covered by the Community Care Program. Please click below to verify your provider’s coverage.
