Free Sports Physicals

CMH Raiter Family Clinic is offering free sports physicals to local athletes who intend to compete during 2026-27.

Who is required to have a sports physical?

The Minnesota State High School League (MSHSL) requires students in grades 7–12 to pass a pre-participation sports physical before they can practice or compete. By law, these examinations are valid for three years from the exact date of the exam. Completed forms must be signed by a medical provider and submitted to your school’s athletic office

Do I need an appointment for a free sports physical?

No, you don’t need an appointment. Free sports physicals are offered on a first come, first served basis.

Will I be charged for the exam?

No, sports physicals offered on July 29th, August 5th, and August 12th are free of charge. Health insurance is not required and will not be requested during check-in.

Do athletes need to be accompanied by a parent/guardian?

All athletes under the age of 18 should be accompanied by a parent or guardian.
