Free Sports Physicals

Pre-registration for this event has closed. Still need a sports physical? Walk-in registrations are welcome beginning at 5:30 PM on Tuesday, June 18th. 

CMH Raiter Family Clinic is excited to offer free sports physicals to local middle school, high school, or collegiate-level athletes who intend to compete during 2024-2025.

WHEN: Tuesday, June 18th
TIME: 4-7 PM
WHERE: 417 Skyline Blvd., Cloquet, MN

The Minnesota State High School League (MSHSL) requires all athletes in grades 7-12 to have a current (within the last three years) sports qualifying physical on file with their school.

A sports physical is not intended to replace annual and/or as-needed visits to a primary care provider. To schedule a comprehensive well-child visit, please call 218-879-1271.

Will I be charged for the exam?
No, all sports physicals offered during the event are free of charge. Health insurance is not required and will not be requested during check-in.

Do athletes need to be accompanied by a parent/guardian?
All athletes under the age of 18 should be accompanied by a parent or guardian.

Who will be conducting the exam?
CMH Raiter Family Clinic’s team of physicians and nurse practitioners will be conducting exams.

Do I need to bring anything with me to the exam?
If you would prefer to complete the sports qualifying physical exam form before your physical, download and print the form here. Bring the completed form with you to your exam.
