Transitional Care
Transitional care—sometimes called “swing bed”—is for people recovering from a serious illness, injury, or major illness who are not quite ready to return home or to their previous level of independence.
Who qualifies for transitional care?
Transitional care is a program for patients who no longer require around-the-clock care but are not yet well enough to return home. Patients who complete a transitional care program are less likely to be readmitted to the hospital with new complications from the same illness or injury.
Good candidates for transitional care include:
- Patients recovering from complex surgery
- Patients recovering from a serious illness or injury, such as a heart attack, stroke, or traumatic brain injury (TBI)
- Patients with a higher risk of developing complications due to chronic disease(s)
- Patients who require ongoing specialized treatment, such as wound care, rehabilitation services, or intravenous medications
