CMH is a participating provider in many private and public insurance programs. We ask for your cooperation in providing all the insurance information needed prior to registration. If you have insurance coverage, we will submit a claim to your plan for payment of the services. Any balance not covered by insurance is the responsibility of the patient. If insurance payments have not been received within 45 days of billing, account payment becomes the patient or guarantor’s responsibility.
You are responsible for providing any information requested by your insurance provider for prompt payment of the claim. If you choose not to provide additional information, you will be responsible for payment of the balance.
If you have no insurance coverage, a deposit is required prior to or at the time of registration. The balance must be paid in full within 10 days of receiving the itemized bill. Patients without coverage will receive a discount on total charges.
We understand that an unexpected medical debt may create a financial hardship for some people, and they are unable to pay their balance in full. If you are unable to pay the “Current Amount Due From Patient,” we expect the Minimum Monthly Payment according to schedule. If you are unable to pay the minimum amount on your account(s), please contact our Business Office Representative at the contact information listed above.
CMH’s Community Care Program is available to uninsured individuals unable to pay for their medical services. Eligibility is determined by measuring family income and assets against current guidelines. Some services may not be eligible under this program.
Any hospital expense incurred as the result of a vehicle accident, public liability or workers’ compensation will be considered the personal responsibility of the patient. We do bill insurance and will do what we can to help you collect the claims. However, if the insurance claim is denied or payment delayed, the account becomes self-pay and is due in full.
If you have made a credit/debit card payment via the online portal and it is determined that you are due a refund, Community Memorial Hospital will issue a refund to the original card used for payment.
Refund requests or canceling a payment can be made by contacting the CMH Business Office at (218) 499-6721. Please allow 7–10 business days for a refund to appear on the cardholder’s account.
