Visa, MasterCard, Discover and American Express cards are accepted. You can pay your bill online here.

Patients without insurance

If you have no insurance coverage, we require a deposit prior to or at the time of registration. The balance must be paid in full within 10 days of receiving the itemized bill. Patients without healthcare coverage will receive a discount on total charges. Other payment arrangements will be considered if you call Patient Accounts and discuss them with us at 218-499-6721.

CMH provides Financial Assistance for those unable to pay for medically necessary care. Read our policy here.

Community Care Program

The CMH Community Care Program is available to individuals who are uninsured and unable to pay for their medical services. Eligibility is determined by measuring family income and assets against current guidelines. Some services may not be eligible under this program. Patients with insurance coverage

We ask your cooperation in furnishing all of the needed insurance information prior to registration. If you have insurance coverage, we will submit a claim to your medical plan for payment of the services. Any balance not covered by insurance is the responsibility of the patient. If insurance payments have not been received within 45 days of billing, payment of the account becomes the responsibility of the patient or guarantor.

You are responsible for providing any information requested by your insurance carrier for prompt payment of the claim. If you choose not to provide additional information, you will be responsible for payment of the balance.

Patients covered by government programs

If you are covered under Minnesota Care, Medical Assistance, Medicare, or a replacement plan, a current identification card will be requested at the time of registration. If you are eligible for Contract health services, you must present the referral form or card at registration.

Accidents and injuries

Any hospital expense incurred as the result of a vehicle accident, public liability or workers’ compensation will be considered the personal responsibility of the patient. We do bill insurance and will do what we can to help you collect the claims. However, if the insurance claim is denied or payment delayed, the account becomes self-pay and is due in full.

Social Worker

CMH’s social worker’s services include discharge planning, depression screening, cognition screening, monitor adjustment issues, information on health care directives, emotional support and referrals to community resources.

Masking Policy Effective April 2023

Current community transmission levels of COVID-19 are low for Carlton County. In accordance with this data, all CMH settings have moved to a mask optional policy for all patients, visitors, and healthcare providers.

CMH’s Infection Prevention Department closely monitors the status of COVID-19 transmission levels and any future changes to this policy will be made and announced here.

Masks are available at every entrance to CMH facilities and may be worn at any time.
