Community Memorial Hospital is a founding member of Headwaters High-Value Network, a clinically integrated network of 19 hospitals and 50+ clinics across the state of Minnesota.  

This network supports a combined population of more than 750,000+ patients and supports independent healthcare facilities to continue doing what they do best: providing personalized, quality care.  

Headwaters High Value Network provides collaborative opportunities and resources that support CMH’s goal of strengthening availability, affordability, and quality of rural healthcare.

Community Health Needs Assessment

To fulfill our mission and vision as a nonprofit healthcare facility and meet the requirements enacted by the 2010 Patient Protection and Affordable Care Act, CMH conducts a Community Health Needs Assessment (CHNA) once every three years.
