The Transitional Care Coordinator is essential in managing and coordinating care for patients using post-acute swing bed services. This role involves collaboration with interdisciplinary teams to ensure seamless transitions, high-quality patient care, and compliance with regulatory standards. The ideal candidate will exhibit strong communication skills, critical thinking abilities, and a dedication to patient-centered care. Key responsibilities include maintaining consistent swing bed/Transitional Care Program processes, developing and maintaining relationships with outside facility partners, assessing individual patient treatment needs, developing and evaluating treatment plans, facilitating interdisciplinary approaches, and monitoring overall program outcomes. The coordinator will also facilitate admission of new clients by reviewing records, conducting orientations, and ensuring all patients receive comprehensive and personalized care throughout their stay.
Requirements: Current state licensure as a Registered Nurse (RN). Case Management certification (CCM) preferred. Requires knowledge of medical terminology, insurances processes, and healthcare systems. Previous healthcare experience in nursing; bedside acute or skilled nursing. Strong interpersonal and communication skills Excellent organizational and time management abilities. Proficiency in electronic health records and documentation systems. Knowledge of healthcare regulations and compliance standards.
