As a pharmacist you dispense prescribed medications, drugs, and other pharmaceuticals for patient care by performing the following duties. Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and appropriateness. Interprets and implements formulary policy. Monitors medication profiles to ensure patient safety. Participates in Antimicrobial Stewardship Program. Involvement in preventing Drug Diversion. Compounds medications, when needed, using standard formulas and processes such as weighing, measuring, and mixing ingredients. Verifies mixing techniques to ensure established quality control standards are met with emphasis on accuracy of components, labeling process and adherence to infection control procedures. Follows USP 797 guidelines and cleanroom procedures. Is familiar with nursing policies and procedures regarding drug administration. Participates in all aspects of departmental QA. Directs pharmacy technicians engaged in mixing, packaging, and labeling pharmaceuticals. Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals. Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs. Responsible for medication reconciliation for new patients, antibiotic monitoring, attending daily interdisciplinary meeting. Promotes a positive work environment.
Requirements: Licensed as a registered pharmacist by the MN Board of Pharmacy. Previous hospital pharmacy experience with computers, unit doses, IV admixtures and pharmacokinetics preferred.
