The Care Coordinator is essential in managing and coordinating care for patients receiving primary and specialty outpatient services. This role involves collaboration with interdisciplinary teams to ensure high-quality patient care, and compliance with regulatory standards. The ideal candidate will exhibit strong communication skills, critical thinking abilities, and a dedication to patient-centered care. Key responsibilities include identifying and assessing patients with complex care or Social Determinants of Health (SDOH) needs, assisting patients in identifying and addressing barriers to receiving timely, appropriate care, managing all aspects of the COPD Care Coordination Program, providing patient education and support, coordinating care between CMH departments and outside facilities to ensure ready access and transfer of information, and monitoring overall program outcomes.
Requirements: Current state licensure as a Registered Nurse (RN). Case Management certification (CCM) preferred. Requires knowledge of medical terminology, insurances processes, and healthcare systems. Previous experience in ambulatory/outpatient nursing required. Care coordination or case management experience preferred. Strong interpersonal and communication skills. Core values consistent with a patient- and family- centered approach to care. Excellent organizational and time management abilities. Proficiency in electronic health records and documentation systems. Knowledge of healthcare regulations and compliance standards.
