Walk-in Athletic Physicals

The Minnesota State High School league requires all athletes to have a sports physical completed and on file within the last three years. Make sure your child is eligible to compete this fall by completing their sports physical at our convenient walk-in clinic.

WHEN: July 11, 2023 from 4-6PM
WHERE: 417 Skyline Boulevard, Cloquet, MN
COST: $35 per athlete

No appointment is necessary. Open to CMH and non-CMH patients.
