CMH Raiter Family Clinic’s Anticoagulation Management program is for patients taking blood thinners (anticoagulants) such as Warfarin (Coumadin).  

Warfarin (Coumadin) is a prescription medication taken to prevent and treat blood clots. The medication slows down how quickly your blood clots, which helps to prevent or treat:  

• Deep vein thrombosis (DVT), a blood clot that usually occurs in your leg 
• Pulmonary embolism (PE): a blood clot that travels to your lung(s) 
• Arterial thromboembolism (ATE): a blood clot in an artery 
• Stroke: a blood clot that blocks a vessel in your brain 
• Heart attack: a blood clot in an artery that supplies blood to your heart 

Why is anticoagulation management necessary?  

Anticoagulation management is necessary to ensure each patient receives the dose that is safe and effective for their body 

If left unmanaged, taking anticoagulants such as Warfarin can cause serious side effects such as severe bleeding or blot clots.  

What should I expect at my first anticoagulation management appointment?  

At your first appointment, our anticoagulation nurse manager will:  

• Educate you on anticoagulants and their risks 
• Create an individualized dosing plan 
• Conduct laboratory testing, if needed 
• Explain any other medications you are taking & their potential interactions  
• Provide guidance about medication management for any upcoming surgeries or procedures 

You’ll leave your first appointment with increased knowledge of your medications, which can be overwhelming. Our nurse manager is available to answer questions via phone call anytime during business hours.

Living with diabetes can be challenging. Our team of primary care providers and Certified Diabetes Education Specialists help patients manage their diabetes so they can live a life they love.  

Currently, all CMH Raiter Family Clinic providers provide diabetes care, including Type 1, Type 2, gestational diabetes, and pre-diabetes. They work closely with our Diabetes Education team to create an individualized plan of care, which may include: 

• Check-in visits every 3-6 months 
• Instruction on blood sugar monitoring 
• Education on nutrition and meal planning 
• Counseling on lifestyle changes to lower your risk for long-term complications 
• Education on insulin pumps, continuous glucose monitoring, and injection devices

Diagnostic imaging allows your primary care provider to look inside your body’s structures and determine the cause of your illness or injury quickly and accurately.  

Diagnostic imaging is conveniently available at CMH Raiter Family Clinic, including:  

• Standard x-ray, which takes two-dimensional images of tissues and structures inside your body. X-rays are commonly used to detect bone fractures or pneumonia.  
• DEXA Bone Density Scan, which measures the density of calcium and other minerals inside your bones. DEXA is commonly used to diagnose osteoporosis, or brittle bones.  

If you require more advanced imaging, you will be referred across the street to the hospital’s full service diagnostic imaging department.

Your Immunization Records 

CMH Raiter Family Clinic coordinates with the Minnesota Immunization Information Connection (MIIC) to ensure all immunization records are confidential and easy to access.  

Pre-Kindergarten Immunizations 

Minnesota law requires children enrolled in school to be immunized against certain diseases unless your child is medically or non-medically exempt. Required immunizations include Hepatitis B, DTaP/DT, Polio, MMR, and Varicella.

CMH Raiter Family Clinic’s on-site laboratory provides convenient, timely, and accurate lab tests.  

The lab offers a variety of routine and specialty tests to ensure you get the right diagnosis and treatment. Commonly performed tests include:  

• Complete Blood Count: a blood test that measures the amount and sizes of your red blood cells, hemoglobin, white blood cells, and platelets 
• Urinalysis: a test that can assess many different aspects of your health with a urine sample, including liver disease, kidney disease, diabetes, or urinary tract infections (UTIs) 
• Lipid Panel: a test that monitors and screens for heart disease through measurements of cholesterol levels and triglycerides 
• Comprehensive Metabolic Panel: a test that measures different substances in your blood and provides information about your body’s chemical balance and metabolism 
• A1C: a test that measures the amount of sugar in your blood, which is used to diagnose prediabetes and Type 2 diabetes 

CMH Raiter Family Clinic’s Population Health Team includes registered nurses who specialize in helping you navigate Medicare/Medicaid specific services, including Annual Wellness Visits and Chronic Care Management.  

What is a Medicare Annual Wellness Visit?  

A Medicare Annual Wellness Visit (AWV) is not a physical exam. Rather, it’s an opportunity to look at the ‘big picture’ of your health and plan for the future. Your visit will include:  

• An update of your personal and family health history.  
• A review of your current prescription and over the counter drugs 
• A review of previous lab screenings, such as testing for diabetes, prostate cancer, heart disease/cholesterol, or cervical cancer 
• A review of your previous diagnostic screenings, such as mammography, colonoscopy, and DEXA bone mass 
• A review of your vaccination history and education about vaccine options 
• Screening for depression and anxiety, evaluation of fall risk, and cognitive function test 
• Advance care planning, as appropriate 
• Measurement of your height, weight, and blood pressure 

Who is eligible for an AWV?  

You are eligible for an AWV if:  

• You are enrolled in Medicare 
• You have not received an AWV in the past 12 months

• Please contact your pharmacy directly to request a refill. They will send us an electronic copy of your request.  
• We kindly request 3 business days for any medication refills.  

Medication Management Appointments

Cardiology
Better heart health happens here. CMH Raiter Family Clinic partners with Aspirus St. Luke’s Cardiology to offer cardiac care.  

What we offer: 

• Monthly in-clinic visits with a cardiologist from St. Luke’s 
• EKGs (electrocardiogram), a non-invasive test that records the heart’s electrical activity 
• Holter Monitors and Cardiac Stress Tests can be ordered and completed at CMH 

Dermatology
We partner with Twin Ports Dermatology to offer skin care. A Twin Ports Dermatology provider visits patients at CMH Raiter Family Clinic twice a month.  

No referral is required.

Foot & Ankle
Northern Foot & Ankle Associates of Duluth specializes in treating foot and ankle pain for people of all ages. Licensed podiatrists Stacey Nipp sees patients at CMH Raiter Family Clinic twice a month.

Dr. Carly Ritchie offers pregnancy care through 32 weeks gestation. Prenatal care is routine monitoring of both you and your future baby’s health throughout pregnancy.  

When do I need to start having prenatal appointments? 

Prenatal care is important as soon as you know you’re pregnant. 

What happens at an appointment?  

• A physical exam to measure the development of your fetus, including listening to the fetal heartbeat 
• Lab testing to check for gestational diabetes and monitor any chronic illnesses you had before becoming pregnant  
• Guidance on lifestyle changes during pregnancy, including appropriate diet and exercise modifications 
• Recommendations for treatment of common pregnancy related symptoms, including nausea, heartburn, aches and pains, and fatigue 

Where will I deliver my baby?  

Dr. Ritchie will work with you to choose a trusted obstetrical provider for your labor and delivery. Immediately following delivery, both you and your baby may continue care with Dr. Ritchie.
