Your Immunization Records
CMH Raiter Family Clinic coordinates with the Minnesota Immunization Information Connection (MIIC) to ensure all immunization records are confidential and easy to access.
Pre-Kindergarten Immunizations
Minnesota law requires children enrolled in school to be immunized against certain diseases unless your child is medically or non-medically exempt. Required immunizations include Hepatitis B, DTaP/DT, Polio, MMR, and Varicella.

Dr. Carly Ritchie offers pregnancy care through 32 weeks gestation. Prenatal care is routine monitoring of both you and your future baby’s health throughout pregnancy.
